Selecting the appropriate needle type and technique based on the type of tissue, lesion size, imaging method, and diagnostic objective has a direct impact on sample quality, the level of pain and complications, and the speed of diagnosis. In recent years, biopsy needle manufacturers have focused on reducing invasiveness, improving sample quality, and making the procedure easier for operators. The Italian company H.S. Hospital Service is one of the leading manufacturers in this field, offering a range of biopsy needles for both soft and hard tissues.

Types of Biopsy Needles

Biopsy needles can be classified according to three main criteria:

Purpose of sampling (cytological or histological)

Sampling mechanism (manual aspiration, core sampling with a cutting blade, or vacuum-assisted sampling)

Type of target tissue (soft tissues such as the liver, breast, and thyroid, or hard tissues such as bone and bone marrow)

Each type of biopsy needle has a different design, gauge, tip, and tissue acquisition mechanism, all of which determine sample quality and the risk of complications after biopsy.

1. Fine Needle Aspiration (FNA)

These are thin needles, usually available in 22 to 25 gauge and sometimes 21 gauge, and are used to aspirate and collect cells and fluids. A 2 to 10 mL syringe can also be attached to the needle hub so that the sample can be collected through suction. These needles are commonly used for sampling the thyroid, lymph nodes, subcutaneous masses, and certain lung and breast lesions for initial diagnosis.

The advantages of FNA needles include minimal invasiveness, low cost, rapid sampling, and a low risk of bleeding. However, they generally do not preserve the histological architecture of the tissue and may therefore be insufficient in cases where histopathological diagnosis is required, such as evaluating the structural details of a tumor.

2. Core Needle Biopsy (CNB)

These are thicker needles, usually ranging from 14 to 18 gauge, and are designed to remove a cylindrical core of tissue. Core biopsy needles are available in semi-automatic and fully automatic versions and are used to obtain tissue samples while preserving their histological architecture. They are commonly used for biopsies of the breast, liver, and certain internal solid lesions, usually under ultrasound or CT guidance.

The main advantage of core biopsy needles is that they provide larger and more suitable samples for histological and immunohistochemical evaluation. Their disadvantage is that they are more invasive than FNA needles and carry a greater risk of bleeding and pain.

3. Vacuum-Assisted Biopsy (VAB) Needle

This needle uses suction to obtain multiple large tissue samples through a single insertion site and contains an internal cutting blade and a vacuum channel for cutting and aspirating the samples. It is suitable for removing a larger volume of tissue, reducing the need for diagnostic surgery, retrieving microcalcifications, and decreasing sampling errors. It is commonly used for breast biopsies and lesions that require larger tissue samples or the retrieval of microcalcifications. Studies have shown that, in some cases, it provides greater diagnostic accuracy and a lower false-negative rate than core needle biopsy. However, it requires dedicated equipment and, in rare cases, may result in a larger hematoma.

4. Image-Guided / Stereotactic Needle

These needles have depth markings or echogenic areas on their shafts so that they can be clearly visualized during ultrasound-guided or stereotactic procedures. This feature helps improve accuracy during image-guided biopsy procedures.

5. Endoscopic Ultrasound-Guided (EUS) Needle

These needles are available with a variety of tip designs, including Lancet Tip (similar to a conventional injection needle but sharper), Franseen Tip (similar to a three-pronged spear), Fork-tip (similar to a small fork), Reverse-bevel (a simple tip with a cutout in the needle wall), and others. The tip is designed to allow biopsy of internal organs such as the pancreas and liver through an endoscope. Studies have shown that Franseen and Fork-tip needles often perform better than conventional FNA needles under many conditions and provide larger and more adequate tissue samples.

6. Coaxial Needle System

A coaxial needle system consists of a large outer cannula through which a smaller biopsy needle is inserted. Its main advantage is the ability to obtain multiple tissue samples through the same access route, while reducing the risk of bleeding and the spread of cells along the needle tract. These needles are commonly used for deep biopsies of organs such as the liver, kidney, and lung.

7. Bone Biopsy Needle

A bone biopsy needle is a strong, hollow metal instrument used to obtain samples from bone tissue or bone marrow for the diagnosis of conditions such as bone cancer, metastases, osteomyelitis, and blood disorders. The needle has a sharp cutting tip and an internal stylet that enables penetration into the bone and tissue sample collection. Its advantages include minimal invasiveness, high accuracy, lower cost compared with open surgery, and the ability to be used under imaging guidance such as CT or fluoroscopy. Potential disadvantages include pain, mild bleeding, needle breakage in very hard bone, or an insufficient sample volume in some cases.

H.S.
